    Title of Survey: Screening at Ambulatory Surgery Centers
    Principal Investigator: Nicholas Heiser, MD

    Dear Participant,

    The purpose of the study is to assess common practices among anesthesiologists with respect to screening patients for surgery at an ambulatory surgery center.  We are trying to identify which patient comorbidities are used to exclude patients from ambulatory surgery.  It is our hope that the results will help guide decision-making as ambulatory surgery at such facilities continues to increase in prominence. The survey should take about 10 minutes to complete.  The identity of respondents will be kept confidential. IRB 461-17-EX.
